You are a Community First Responder tasked to a 70-year-old female, Evelyn, who has fallen in her home. The call information states: “Fall, possible leg injury, unable to stand.”
You arrive at a small terraced house. Evelyn is found in the living room, lying on the floor next to an armchair. The scene is safe. She is conscious, alert, and in noticeable pain. The room is very warm, the house is clean, and the living room TV is showing a repeat of bargin hunt from 2007, on full volume.
History
She tells you she tripped over the rug whilst getting up from her chair. When she fell, she landed on her left side and hasn't moved since due to the pain.
She denies hitting her head and denies loss of consciousness. She has a Past medical history of Hypertension, osteoporosis, chronic kidney disease stage 4 and 'Mild COPD'. She is prescribed Amlodipine, salbutamol and co-codamol. She lives alone with no cares, but family visits three times a week.
Upon Assessment
There are no A-E concerns of a life-threatening nature. She reports severe left hip pain, especially when she tries to move it. From what you can see, the left leg appears shortened and externally rotated. She cannot lift or stretch her leg. Pain increases when you try to move her. No contusions are present around the painful area. The patient complains of nausea and mild lightheadedness. She appears pale and a bit clammy, as well as anxious.
Observations
Respiratory rate, 20, regular
HR: 88 (R) with matching pulse rate
SpO₂: 94% on room air
A blood pressure of 139/87
A Blood Glucose measurement of 4.6 mmo/l
A Temperature of 37.9°C
a perifral and capillary refill time of 3 seconds
You now have to make a plan for how you are going to help Evelyn off the floor. You are equipped with a standard Community First Responder kit, including a lifting cushion (or riser chair) and methoxyflurane. You are able to contact Control via a PDA phone call, who can provide additional resources at different acuities. What would you do?
